1. Call your insurance company to verify your outpatient physical therapy benefits.
2. Bring with you your referral for physical therapy that is signed by your physician and your insurance card.
3. Fill out and bring with you the patient registration form.
4. Review, sign and bring our Notice of Privacy Practices.

We accept most major insurance plans, (Regence, Premera, Group Health, Medicare, L + I etc.) however, it is important to contact your insurance company to verify your individual physical therapy benefits.
