Please call our office at (360)714-0870 to confirm other insurance plans.

Our team is experienced with every aspect of insurance approval, verification, and billing. While some insurance companies require complex navigation, rest assured that we will do everything in our power to ensure that you receive the coverage and benefits that you deserve. We verify every patient's benefits before their first appointment, and coordinate with your insurance company to maximize your coverage.

Our goal is to make physical therapy as accessible as possible for everyone. From complex insurance issues to the simple questions, we are always available to offer guidance. Call our front desk at (360)714-0870, or email us at: info@performancephysicaltherapy.com.

Co-payments are due at time of service if applicable. If you have valid insurance, we will bill any remaining balance to you after your insurance company has processed your claims.

Direct Access/ Self Referral

Direct Access is a policy that allows you to see a physical therapist without a physician's referral. This can save you time, money, and ultimately get you back what you enjoy doing as fast as possible.

A majority of the commercial health insurance plans listed above allow you to self refer yourself to outpatient physical therapy. That means that no referral is necessary for you to schedule a physical therapy evaluation.

You may ask, is direct access safe? The answer is Yes! Physical therapists are trained to recognize “red flags” that may signal the need for more invasive medical intervention. In those cases, referral to your physician or healthcare provider is made immediately by the PT. If you are suffering from a musculoskeletal condition that causes functional movement limitations, use your best judgment when deciding which healthcare practitioner to see. A visit to your local physical therapist is a safe place to start on the road to recovery.

To verify if your insurance plan allows for this, our recommendation is to call the customer service phone number on the back of your insurance card. Simply ask a representative if your insurance plan allows you to self refer to outpatient physical therapy.

If you have direct access available to you, all you need to do is request an appointment with us and we will give you a call to book your evaluation!

Self Pay/Cash Pay

Cash pay physical therapy is a model of physical therapy in which a patient pays the physical therapy practice directly for their services at a discounted rate, instead of relying on insurance reimbursement. Cash pay physical therapy requires no referral, has no visit or treatment limits, and offers the fastest and easiest way to receive physical therapy care.

This is typically the best option for those without insurance coverage, or patients who have a unique insurance plan that we are not in-network with. Regardless of where you receive physical therapy treatment, know that if you do have an insurance plan that clinic is contracted (in-network) with, cash pay is prohibited and the clinic is obligated to bill your insurance.

At Performance Physical Therapy, we have implemented cash pay physical therapy for several reasons:

Lower Overall Cost - Cash Pay pricing is a lower cost than typical insurance billed treatment, resulting in money saved for those who do not have insurance coverage for various reasons.

Price Transparency - With fixed rates, patients are fully aware of the cost of their treatment. This allows for less surprises and better financial planning.

Control Over Services - Commercial Insurance companies have the power to put a stop to your physical therapy treatment at any point they deem necessary. Cash pay physical therapy allows you to receive care based upon your needs and not insurance restrictions.

Pricing for Self Pay Physical Therapy

The following prices are for full 40-minute 1-on-1 physical therapy appointments with your physical therapist. If you have a treatment plan that will require continued physical therapy, ask us about our superbill program!
